WARNING: The following contains spoilers for The Good Doctor Season 4, Episode 19, “Venga,” which aired Monday on ABC.

Shaun and Lea’s relationship on The Good Doctor has been a rollercoaster, to say the least. The couple only started dating at the beginning of this season, and they’ve already been through their fair share of heartbreak. After learning she was pregnant and preparing for parenthood, Lea suffers a complication and loses the baby, leaving her and Shaun devastated. Now, after spending the last few episodes working through their issues, the couple heads to Guatemala, where things take a turn for the worse.

The trip begins as expected, with Shaun being eager to tackle new medical mysteries and Lea just happy to be out of the apartment. But as things progress, Lea is left alone to wander the hospital, and she passes by the maternity ward where she sees a new mother and her child. Being reminded of her child causes Lea to break down, but she doesn’t want anyone to know how shes’ truly feeling.

Later that night, the group heads to a bar to celebrate their first day. Everyone seems happy, Shaun especially, but Claire notices Lea isn’t her usual talkative self. Of course, Claire wants to talk to her friend, but she gets a call from the hospital that forces her to leave. As Shaun and Claire check up on the patient, Lea heads back to the maternity ward. She gives the mother a pair of socks gifted to her for her child, which the woman graciously accepts. Sadly, when the woman asks Lea if she wants to hold the baby, Lea declines, not wanting to hold a child that isn’t hers.

Devastated, Lea goes to sit on a bench alone, only for Claire to show up with news about the patient. Unable to hide her emotions, Lea breaks down in front of Claire, who tells her she needs to tell Shaun how she’s feeling. Back at the hotel, as Shaun gets ready for bed, Lea takes Claire’s advice and tries to explain the situation, but all Shaun wants to do is talk about his upcoming surgeries.

While she eventually gets through to him, it’s clear it’s too little, too late. Lea explains to Shaun that her parents want her to go home to Pennsylvania to help her work through her issues, and she plans to take them up on the offer. Unfortunately, while that may be good for Lea, the last time she went home, Shaun became a shell of himself, and now, the couple’s future is up in the air going into the final episode of Season 4.

Created by David Shore, The Good Doctor stars Freddie Highmore, Antonio Thomas, Richard Schiff, Paige Spara, Fiona Gubelmann, Will Yun Lee and Christina Chang. New episodes air Mondays at 10 pm ET/PT on ABC.
